1. Lookup a Patient using patient icon.
2. Click the template icon and open the 'Packet Navigation' template from the list.
3. Add the patient’s program to the program field.
4. Click to open the 'Child and Adolescent Needs and Strengths (CANS) Assessment, 5 to 17' (BH) template from the Packet Navigation.
5. Click in the radio button that best fits the 'Reason' for this visit.
6. Click 'yes' or 'no' if the client has 'Medicaid'.
7. Click in the 'Performed Date' field and use the calendar popup to change the date if needed.
